QuDoS multiple sclerosis: NHS case studies 2019 Team of the year commendation

Hertfordshire Neurological Service, Central London Community Healthcare NHS Trust (formerly West Herts Community Neuro Team Central London Community Healthcare)

This team is made up of nurses, physiotherapists, occupational therapists, clinical psychologists and rehabilitation assistants. The interdisciplinary group meet regularly to triage and discuss complex cases. Patient visits are often planned in collaboration with carers, health and social care, district nurses and mental health team colleagues. The team focusses on holistic patient care to ensure that those with complex cases aren’t side-lined between services. They have developed specific care pathways for those with additional needs (eg, vestibular dysfunction, hip replacement), and have initiated a standing frame loan service, which encourages patients to stand daily in their own homes. The team has also formed close links with their local gym, initiating joint exercise groups in their ‘toning suite’, and links with other groups (eg, local charities and businesses), which has expanded their patient pool. Staff have reported increased satisfaction and reduced burden by working as part of this focussed, dedicated and patient-centric team.
